 * Multiple security failures, a 'distracted' guard and a gate malfunction together led to a massive breach at Joint Base Andrews on February 4
 * An unauthorized man was able to enter the base, roam freely for five hours and then board a C-40 military aircraft
 * The military base is home to the presidential fleet including Air Force One
 * The Department of the Air Force Inspector General released its report Thursday
 * The heavily-redacted report reveals the man was in civilian clothing and had a bright red or pink cap with 'balls on top that looked a little like mouse ears'
 * It cites three key security failures: a guard did not check he had credentials; a faulty gate to the flight line; and he was not challenged when he boarded plane
 * The Air Force maintained there is no evidence the perpetrator planned to cause any harm with Lt. General Sami Said saying he 'just wanted to see airplanes'

Quite what had distracted a guard into not noticing the man wearing a bright hat with 'mouse ears' as he drove onto one of the nation's most secure airbases may never be known.

That question - plus the mystery identity of the unauthorized intruder who enjoyed five hours freely exploring Joint Base Andrews before boarding a government aircraft last month - still remains unanswered after the Department of the Air Force Inspector General released its report Thursday into the massive security breach.

On February 4, an unarmed intruder entered the military base that is home to the presidential fleet including Air Force One.
